Publicity Form

This form is used to collect and collate all of the marketing materials that will be needed for your upcoming work at Young People’s Theatre. There are four components below: the questionnaire, the headshot, the kid picture and the bio. If you do not have all the materials at once, please submit whatever you have available. If you need to submit other items later, you will need to fill out the mandatory fields, but you do not need to resubmit items that have already been sent.

Please upload an 8x10 inch black and white or color image, full face shot. Ensure that your head takes at least two thirds of the entire image. Only jpg, tiff or pdf file formats are allowed, any other file format will not be accepted.

Please attach your bio in a Word document, if possible. If you do not have a prepared bio to attach, please type your information using these guidelines below. (maximum of 100 words)
* Bios will be edited by YPT for length and continuity, if necessary, and submitted for your approval. Theatre credits will take priority.
* Please list the title of the show in italics, followed by the theatre company or production company in brackets - example: Hana’s Suitcase (YPT). If you have performed multiple shows for the same company, please list them all before that company name – for example: Hana’s Suitcase, The Monster Under The Bed, Seussical (YPT).
* Please list your YPT credits first, followed by other theatre credits, then TV and film credits next, and finally any miscellaneous credits (education, membership, awards, music credits, etc).
